Google search says 2 things, forum results in MAF being the problem, but then theres this:

A 734 fault code, 4th. gear out of ratio, is one of a couple codes that when they show up together will indicate the MAF is dead. The other indicator is if you get both banks running too rich or too lean, then it is a MAF for sure.

734 is a transmission ECU code, and :

P0734 is detected when A/T cannot be shifted to the 4th gear position even if electrical circuit is good.

So that would explain high RPMs. Time to focus on tranny (fluid level, fluid and filter change, etc.)

My shift points and idle are a mess also when cold in the morning. On start up it races up and then sits about 1000 rpm, when I get going it will run about 2500 rpm at 65 and isn't kicking into 4th gear. Once it warms up it goes into 4th with no issues and runs about 2000 rpm at 65.
So MAF or tranny fluid? Or both?

Neither, more than likely. Savanna will surely jump in with a quote from the RAVE, as I am too tired from work to find it there, but I know the torque converter does not lock up until the engine has reached operating temperature, and the same may be true about 4th gear also.

Driving home from work, I only take the highway if the engine is at normal operating temperature. Normal roads if not. My work is about 3 minutes from the highway on ramp, and I am only on the highway for a couple of miles. I would just rather keep the revs below 2000.

On cold startups, mine starts at 1,200 RPM, then drops to 1,000 in less than 30 seconds, and then to 750~ after a minute or so. That is normal.

On the D2, the tranny ECU gets all sorts of inputs from the ECU and other sensors, and computes when shift points should happen, and has an adaptive pattern it stores and compares, etc. It will decide when it is best for the machine to shift. On the D1 it is simpler, no 4th gear until about 53 mph.

Guys what's the normal rpm range for idle on the 03-04 4.6? I think I might be idling too high.

Well thanks LM, your situation is an exact replica of mine. If this is normal I will add it to the list right after the other "normal Disco quirks"
1) feathering the gas to avoid banging the tranny around
2) only using the cruise control when going for a longer than ten minute trip on the highway
3) on cold start be patient with the tranny ECU

I am learning more with every little issue.

Well I don't know if it is normal but… my '04 4.6 idles @ 1K RPM for a few minutes on cold start, then down to steady-state at 750. When driving at 60-65 I run about 2000-2250. Unless cold then it is around 2500-2750.

Hmm alright thanks man. I was idling at 1200 at cold start today and settled to about a 1000 rpm. On the highway I could hit 70 mph at about 2500 rpm.
