lol still have it printed out from when i did it... only catch is that the hole you cut in the plate, at least for mine, is almost bolt hole to bolt hole, alot larger then what they show there, and its tough metal for my lil electric dremel lol, ate a few wheels before i decided to drill holes every half inch and use a hammer and crowbar end to punch hole to hole to "cut" it out. shoulda did that from the start, looks like ****, but who cares its under the plastic anyway. AND, it makes the interior very noticibly louder while driving, lots of noise gets up through there now even with all the otehr crap back on, even with the stock foam crap put back in.

When I purchased my 2004 CDL, I was able to purchase the entire plate, the CDL Shifter and the standard gear shift all mounted up to the plate, with the shift *****, boot and P R N D 3 2 1 shift cover.
I simply drilled out the rivets, removed the old plate with all shifter components attached, installed RivNuts to the 2004 plate, and then reassembled everything using button head allen screws so that I could easily disassemble the whole thing in the future if necessary.
Ended up being a really clean installation that way without the need to cut anything...
