clutch noise, any suggestions?

i have a 1994 discovery 300 tdi with 146,000 miles, which i'm having a bit of a problem with, and would appreciate any advice before i go and order any parts.
The problem is when changing gear, as the clutch is released there is a terrible grinding noise, especially when under load. when actually in gear she goes along fine.
i've been told this could be the thrust bearing, does this sound feasable?
Also there has always been some slack in the transmission, ie clunking and bunnyhopping if not driven smoothly, could this be the transfer box? a friend has a TD5 with 155,000miles and similar clunking and has just had to have a new transfer box put in. If i'm likely to have to replace this as well as the clutch, i may as well do this at the same time. is it easier to remove the engine or the gearbox?

It will always be eaisier to remove the transmission than the engine.
Sounds like you need a new clutch, and while you are in there replace the throwout bearing and everything else that is associated with the clutch.
As for the transfer case, chaneg the fluid in it, use a full synthetic gear lube, 85w-140 if you can get it.
Clunk noise in the t-case is normal, as the gears wear that creats slack in the gears and that clunk you hear is the slack being taken up.
Change the fluid in both differentials and the t-case every 50,000 miles using a full synthetic gear lube and it will clunk like that for the rest of your life.
Also make sure that you are checking the fluid level in your diffs and t-case every 6 months, more often if you have a leak.
