Here are the part numbers for your Proshaft/Dynamic Drivelines front drive shaft
Proshaft LLC may have went out of business and Dynamic Drivelines is a separate entity now and does not sell the parts needed to perform a rebuild on your drive shaft. I've tracked down the part numbers for the u-joints and Dynamic Drivelines recommended centering ball kit. These part numbers are only for the aftermarket Proshaft/Dynamic Drivelines front drive shaft. Also be advised not to place any orders on the Proshaft website as you will not get your parts or money back.
U-joints: Moog/Precision 369
More Information for MOOG 369
Centering Ball kit: CV Center Repair Kit 404-4
404-0 Spicer Style CV Center Repair Kit
A lesson learned the hard way. If you decide to purchase an aftermarket drive shaft, do your research on the company/manufacturer of the drivelines and make sure they are not about to go out of business and make dam sure there will be product support for the shaft even if the company goes out of business. When you buy make sure you acquire all part numbers from the company. This will save you alot of hassle down the road when its time to rebuild your shaft.
My Proshaft LLC front drive shaft was installed in February 2013. My truck had around 146,000k ish. The shaft started making noises last weekend and the truck currently has around 156,000. I expected this drive shaft to go alot longer before needing a rebuild.
Best of luck to everyone.

True and valid points however it is cheaper to rebuild the shaft yourself. Once in a great while you may need to get the shaft rebalanced but other than that Id rather save money, gain the experience and do it myself. That way I know its done right and with the right parts. Furthermore there are no driveline shops nearby so I really have no choice.
Got a front half shaft from ProShaft, LLC | Driveshaft Supply, about 10 months ago..only put on 15kmiles and it's failing... Can't get Justin to call me back or answer my emails. Does anyone know of another number for them? I'll try Lucky 8....
