Have you verified your temp readings? You can check ECT sensor at manifold compared to a IR thermometer aimed at it, senors not known for exacting accuracy. Possibly sensor output high.

If you still have a overheating issue after all you've changed you may want to pull the block drain plugs and flush your block well if not done prior.

I was just looking up one how to remove the block drain plugs. doesn't look like it will be easy. I use the icarsoft scanner to monitor the temps from the stock sensor. need to get a ir thermometer to check it at the manifold.

Actually not difficult at all from beneath. Have already done it a couple of times with vehicle on my 6" ramps. 7/16" 12 pt socket or combo wrench worked for me. RH side access not great but removing knock sensor plug and moving starter harness out of way allows easy R&R. Just need good light and no fear of getting dirty. Once plugs out easy to flush through manifold outlet pipe.

My first D2 flush was removing lower radiator hose, block plugs and upper hoses. Flushed well with water put back together. Used one gallon of vinegar and balance of water for coolant top-off. Ran it for a couple of days pulled plugs, lower hose, flushed again with water. That drain, flush and vinegar douche dropped cruise temps much more than than the 180 thermostat.

Most people don't realize radiator typical capacity is 1/3 or less of total coolant capacity. Radiators are easy to flush, remove plug or or lower hose, flush from top and stuff comes out. Few go to effort to pull block plugs when draining/flushing thinking unnecessary. Crap has a tendency to settle in lower block cooling passages and will never exit unless properly drained and flushed.

There is a reason the block coolant drain plugs are there.
......

FYI, I just used Teflon tape and to typical pipe thread hand torque when reinstalling plugs versus LR's spec to make future removals easier.

Problem with doing that is space, or the lack of. That outlet pipe isn't very big and its a pretty snug space.

Would probably be easier to bore out the manifold and put it there. That't the route I'm looking at doing.

To me, this is the route I want to take. I was just looking at all the alternatives. I do not have a manifold to bore for thermostat and locating a 3 hole water outlet pipe. I read your linked post, and that is exactly what I want to do.
I see that your post is a month or so ago, have you made progress in this? If so, what thermostat did you find to fit and what outlet pipe will bolt up to the manifold?

Been busy since then with work and playing with my new toy but the weather is getting cooler once again so the toy goes back under her cover. But I have a stock lower manifold sitting in my basement with the attached outlet pipe. Will take pictures tomorrow and update the thread a bit. Still looking for a thermostat to fit, but will measure the space we have to work with.

I'm confused why you're so focused on the thermostat at this point? Obviously there's something else going wrong on your truck. Waterpump, viscous, HG failure?
