I had the same sound. Turns out it was the exhaust gaskets where it meets the manifold. Sounded like lifters or liners. But after the gaskets we’re changed the sound is gone.

Based on the nature of the sound, when I hear it, an exhaust leak (gaskets or otherwise) seems to fit the bill. I'm just struggling to understand why I don't hear it when I rev the engine while standing still.

I've been installing an inline thermostat and flushing my coolant system (auf wiedersehen dexcool) over the past 2 days so I've been revving my engine in neutral quite a bit during the system bleed procedure. Still don't hear the noise unless I am actually driving.

It could just be the location of where the sound is coming from. I currently have a loud groaning/humming that I can only hear inside. I've opened the hood and listened from underneath and I don't hear it like I do inside. I believe it's exhaust related as my muffler isn't aligned correctly.

Could also be cats if they haven’t been changed. Mine were replaced and made a similar groaning sound. The right side cat had blown apart inside. You can test to see if it is cats my hitting each one with a mallet. If you hear a rattling sound then they are likely broken inside

Ah, interesting...I'll need to check that out. Truck only has 89k but I guess anything is possible. How many miles did yours have?

Mine had 126k miles but they were damaged long before that. I just finally decided to get them changed.

Hit mine with a rubber mallet and no clanking sounds inside. Maybe I didn't hit it hard enough...I think it's the muffler I put on recently. The old one was rusty. Bought a used one but it's not seated right at the connection to the cats. There's some condensation leaking at the connection so there's obviously a leak.

Sorry for the thread derailment.

I hit mine with a mallet and no sound as well. I used a stethoscope and confirmed the ticking sound was the drivers side cat. It was soo bad it sounded like a liner was smacking the cylinder head. Replaced both cats and the motor has never sounded better. Highly recommend using a stethoscope. Guess they started going bad at around 120k.

Update. So I think I have found the source of the noise. When I was looking around my engine bay I noticed one of the studs connecting the passenger side exhaust manifold to the pipe leading to the catalytic convertor the other studs look like they are in good shape. I'm waiting on the parts to arrive then I'll see if noise goes away.

Same problem I encountered.
