Default Weak brakes and weird abs codes

Alright so since I bought my rover it's always had this brake issue , ofcourse the three amigos are on which is a whole thing in itself , but the brake pedal when pushed feels really hard and I have weak brakes , no matter how hard you push the pedal you can't get the brakes to lock up or when the abs is working it won't engage that, the stopping distance is laughable , I checked all 4 calipers and lines and they seem fine , what else do you guys think it could be, also the codes I'm getting from my hawkeye are
fault 4012

Vac leak to your booster? Is it a V8 or diesel?

V8 I didn't notice any leaks when I checked

I did the brake booster test I saw online awile ago where you press on the pedal and start the car and everything seemed to check out I will try it again when I'm on break at work but that abs code I got the people from Hawkeye and Atlantic British couldn't explain it
