Its actually stuck in the picture... lol The plows come through our neighborhood and leave these piles of snow all over the place. I can usually be seen in the morning blasting through them with the grill guard. SO MUCH FUN! No damage or anything to the rig, but snow blows up everywhere. Well there was a particular mound a bit bigger than normal. Rammed it and the axle in the front axle got buried and the rear tires were on a bunch ice.... Took about 10 min to get it out. Fun times!

Kinda dealing with the same issue with my 95 LWB as KRAZZ describes. I want to chalk it up to it being 10F or lower, but it seems that the heat should be better than luke warm. I've looked for wetness around the heater core, coolant level is solid and unwavering.
There is a strange gurgling at times behind the dash at times, leading me to fear heater core, but I have found nothing to indicate an issue there. I'll try the hot to cold to hot trick tomorrow morning, then look deeper.
