Default head gaskets blowing

has anyone had any issues with the V8 3.9 litre motor blowing head gaskets soon after a rebuild and what was the cure?

improper installation of the new gaskets,
a cylinder head that wasn't resurfaced or warped,
overheating(again)
or worse a cracked block, slipped-sleeve all come to mind

You mean rtv? Typically you don't need rtv on a rubber gasket but...
