You need a washer and a bushing on each side of the perch.

Already have the provided washer and bushing both above and below the perch (one washer and one bushing above, one washer and one bushing below).

It's too tight then.. the bushing shouldnt be smashed down like that just tight enough that the bushing is squished a tad more than when you put them in.

Fair enough. I have just always been under the impression that bolts should be tight...tight meaning as tight as someone my size can get them with a slight grunt. Thanks.

Yea but you have to take them off and re thread them multiple times to really ruin them.

Here is the pre-lift picture. I was at about 31.5" from the ground to the middle/high point of the fender. I will take a post-lift picture tomorrow. When I measured it was right about 34" to the same point.

It was 31.5 to the highest point of the body line on the fender (just above the tallest part of the tire). Probably not the ideal spot to measure since it is curved, but it is a habit.

Edit: I figured as long as I measured in the same place both times I would know how much lift I got which was ultimately all I cared about since tires would also effect my numbers versus everyone else's...and I have worn down, generic, stock sized tires on there now (I have only owned the truck 3 weeks now).
