I suspect the lack of rigid adherence to coolant maintenance led to most failures BMW changed the thermostat to a much more complicated design from the original inline style. A certain temperature has to be attained by a mixture of hot coolant and cool coolant for that thermostat to work properly. If the coolant gets out of spec, the temperature differential can expand, creating increased heating and cooling cycles to the heads, tty bolts, and gasket.
