Those plugs are super rich (or seriously oil fouled, a little tough to tell based on the light and angle but I am leaning to super rich). Check the 02 readings and see if they read full rich. If I am right you just need to figure out why...

I'm actually glad you asked because I found a picture of mine on the old engine.
I have to find the date on that picture. That may be the one that disintegrated. The empty holes are the bolt holes for the torque converter. Putting those bolts in with the engine on (only way I know to do it) is brain surgery through a hole at the back of the oil pan.
So that picture above is the new one, but prior to swap.

My 4.0 ran rich and the plugs are out of it. I didn't think much about the ones pictured because I did the old man thing and squirted oil in the cylinders after I primed it (so first start up blew some oil). There is maybe 3 minutes run time on those in this engine. But I've resolved to go get some new plugs tomorrow so I'll have a good baseline.

Maybe you sucking in fresh air through the plugged SAI ports which is causing the O2's to call for to much fuel?
I remember seeing some pictures of some welded up SAI fittings and they didn't look good. I don't remember who's they were.
So swapping on NON-SAI heads should eliminate that as a potential problem.

They were probably mine. The plugs are porous. I did blow into them and no air escaped but who knows. My non Sai heads have real plugs in them so I could swap. But...
I have two extra sets of non Sai heads. I know the valves are good, too. I'm pulling the best set tonight, and already have that engine on the stand. I've gotten a little side tracked because I want to move the coils, too. So I have some help unwinding a spare harness while I work on the heads. If my connections are good, I'm going to just make longer plug wires, put the coils on the passenger firewall, swap the heads, and drive away happy.
I am waiting for delivery of some new studs (this engine has bolts), and that should be here tonight. Amazon screwed me on the plenum gasket Delivery so I may, gasp, re use.
Amazon Prime is becoming less and less valuable for two day shipping.

So after owning them for years, I realized I have three different sets of heads.
My oldest heads have no holes for SAI. But those heads overheated badly and need some work.
My medium heads have holes for SAI, but they have official plugs in them.
The newest ones are soft, have holes, and have my welded SAI plugs in them.
I swapped all of the spark plugs for new. No difference. Checked the head plugs and they may look ugly but they are air tight.
Compression is good so I clearly have a problem with coils or wires, or an electronic problem, or my new cam is junk, or maybe my rockers/springs/valves are having an issue. I pulled the medium heads this evening and cleaned them, so I will start swapping things out tomorrow.
Could be injectors but they worked on the other engine
Im really not sure what I'll do tomorrow, but I'll do something.

My welded SAI plugs are pretty ugly too. Could've been either.
My prime was a little rough for a couple weeks, but when I complained they fixed the issue.
I don't remember what I was responding to when I started this. I'm going to go look...
Edit: I honestly have no idea, but studs, non SAI heads, and new plugs sounds like a good start. Man, I really wish I remembered what I meant to type.

Right, that's what I was talking about. Stuff was taking 4-5 days to get to me for a couple weeks. At first it was annoying, but on a couple items it actually cause me problems and I ended up returning them and getting them to pay return shipping. I also mentioned that it has been an issue for a couple weeks. They added a note to my account, and it hasn't been a problem since.
