What is the difference in the sport model vs the standard medium shock?

They don't make a medium model.
There is a standard OME, the H.D. and the new Sport which is the first 3 valve gas shock on the market. Why don't you go do a Google search and just read up on them.
Are you also replacing the coils, if so who's are you using?

Looks to be a nice shock. I was thinking of going with the OME Medium springs and a steering damper also.

I noticed there is one sport model listed for the front and a soft and firm for the back. Which one would be the best bet for the rear? I have very bumpy roads where I live.

Should only be 1 Sport shock for the front and one for the rear, not 2 rear shocks.

I looked at the Atlantic British website and there appears to be two, one Item: OME60038 (soft) the other Item: OME60039 (firm)

You are correct, and whatever you do, don't buy from A.B., go to Lucky 8 and only buy the HD Sport shock. Are you also installing new springs?

Yes, I plan on installing the medium springs. Lucky 8 has the same two models listed and I do not see a HD sport. I emailed ARB to see what firmness the front is since there is only one option.

ARB said to go with the soft unless I had at least 400 pounds of load in the back at all times.

Does anyone have any knowlege on the 2 versions of these shocks they list for the back of the Disco 2? AB now lists the numbers as OME 60025 for the "firm" model and 60026 for the "soft" model. Lucky 8 also list both numbers but doesn't describe anything about them.
