So both of my 95 Disco's have the brake lights without directional indicators (the only rear turn signals are the ones in the bumper).

Would it be possible to rewire (maybe using the wiring for the bumper turn signals) and replace the rear lights with the directional versions?

 

Sure, why not?
My '97 has them in both the bumper and tail light.
Get some newer DI tail lights and rewire man!!

 

Or just get the newer tail lights and move your turn signals from the bumper to the top. More than likely if you change out your rear bumper you won't have the lights in the bumper anyway.
