how do the steering boxes usually fail?

Leaking is probably most common. Getting loose is probably next.

Mine was leaking badly. When I opened it up, there were some pretty deep scratches on the surface of the internal components.

Also on the D2 TD5 the rigid alloy part of the return hose rubs against the corner of the engine sump and wears through. Common fault.
