3. High pressure hose o-ring at the pump. 3% of leaks
4. Front shaft seal - never fails until the front bearing fails (sealed bearing) which does not happen until north of 200k miles. Maybe 2% of leaks.

I can walk you through the rebuild if you still have your old pump.
Thanks @Extinct , I believe it was cause #1, but then couldn't get it to seal again and thought maybe the housing was cracked (nothing visible though). I have been working on it at my buddies place, he is a decent mechanic, just waiting to hear back from him if he has the old pump or not. If he doesn't I am hoping the auto parts shop will refund me for this one and I will pick up one from a wrecker and rebuild. I don't see a rebuild kit on Atlantic British, where do you get the new gasket?

"it looks like the pulley is out of line by about 1/8" so the belt ends up riding on the guide and this led to a failed belt."

I had that happen when I installed the pulley on backwards.

Flipped it around and the pump ran fine.

Thanks for the tip @gulfstream69xr7 , I checked this and it was even worse with the pulley flipped. I wasn't able to get a refund on the pump since I ran it for several months but they are going to send me a replacement. Hopefully that solves this problem for good.

Six months later and another power steering pump failure @#$%.

@Extinct - the link you sent to the new gasket is in Europe, nothing in North America?

@Extinct Got an extra one to sell me so I can pay priority post

@Harvlr I tried one like that and it barely lasted 3 months, the European one looks closer to the original material. Where in BC are you?

I did mine about a month and a half ago. I hope it lasts a lot longer than 3 months. By rebuilding it myself I was hoping to avoid the poor rebuilds that I’ve read about.
