Will this rim work on a 2004 D2?

Rim Diameter (in) 15 in.
Rim Width (in) 8.00 in.
Backspacing (in) 4.000 in.
Offset (mm) -12.00mm
Lug Nut Seat Style Conical seat - 60 degree
Hub Centric No
Center Bore Diameter (in) 3.300 in.
Bolt Circle 5 X 4.75 in

???

No. Your bolt pattern is 5 on 120mm. Different bolt patterns are not interchangeable.

120 millimeters = 4.72440945 inches

So that's a difference of .025 of an inch.

Sanding the paint off the holes would be .025 of an inch.

4.72 inches vs 4.75 inches

I have ? which usually means your not that confident.

With 15" clear the D2 Brakes?

Was is the stock rim backspacing? (4.5", 6"?)

Yes 15" wheels will clear the brakes. If you look you can find the same wheel you're wanting with the correct bolt pattern. Even with the slight difference they will not fit.
