Do you mean that the truck will not roll backwards, or that the trans will not shift into R? Can you drive forward in other gears?
If the low range cable is not adjusted properly, the shift fork could be between L and H. This puts the transfer case in N, but in this case you would lose drive entirely, not just R.

Undo the range shift cable at the case end, and move the shift lever manually to make sure it's in the correct position -- either high or low shouldn't matter. Then try to reverse again. That should confirm whether it's a problem with the cable adjustment.
Also confirm that the CDL is unlocked, since the shifter on the 2004 is connected to both.
