Sudden Acceleration! Prius Problem with 08 LR3!

It was tied to a thread about several folks wanting a NTHSB Warning and recall but the raw count of folks experiencing this stateside is way too low for that kind of attention.
My larger and more personal concern is nobody seems to know the root cause or how to fix it.

Interesting read, be good to know real problem (what it was) and fix. Two vehicles from same dealership, both faulty wonder how close the Vin's were as it may only be a particular range of Vin's
Received my new (used) accelerator pedal assembly today, but heading out to visit family for the next week so won't be playing with it until likely the new year. After reading as much so far as I can on the subject, it hardly seems likely that this condition is due to a component failure. The ECM controls the throttle via changes from the accelerator pedal position (APP), period. The brake switch has a connection to the ECM, so when you are standing on the brakes, the ECM should know this and refuse to increase the throttle setting unless you are also putting your foot into the accelerator pedal. I also have some kind of intermittent problem with my cruise control, in that sometimes it does not go into cruise control mode and sometimes it drops out of cruise control, but this uncontrolled acceleration trouble condition only happens when the vehicle engine is not completely warmed up, and I'm loosely holding the brakes on at a stop. Will do some more reading to see if I can figure out everything that the ECM looks at to initiate the throttle control, although many times some crazy stuff happens to electronics because of grounding or power problems due to corrosion. Will conduct further thought and physical experiments as time allows, but will keep all informed of what's happening...

So this is all I can come up with today, heading out for family stuff for the next week... Hope you all can come up with better stuff/ideas! (my stuff might not be completely accurate)...Check ECM (D131) Pins for V8 and V6
1. The ground for “pedal demand 2“ comes from P Sheet 4 and not the C0635-7 ground that “pedal demand 1” uses (V6)
2. 5V REF 1 C0635-32 and 5V REF2 is C0635-19
3. Pedal Demand 1 is C0635-24 and Pedal Demand 2 is 0635-38
4. Cruise control pins are C0635-35 (-) and 36 (+)
5. Mass air flow sensor on C0634-66 (AFM IAT), 70 is MAF, 18 is MAF GND
6. Throttle position sensor uses C0634-10 as a common ground (SEN GND 3), and C0634-72 as a common 5VDC (5V REF 3)
7. “Throttle +” to the throttle motor is C0634-75 and “Throttle –“ is C0634-74
8. Purge Control Valve is C0634-92
9. On the V8, both accelerator pedal grounds come from the ECU GND 1 is C0635-7 and GND 2 is C0635-8
10. Cruise Control C0635-36
11. Brake Switch C0635-41
12. CAN BUS brake pedal status
So this indicates to me that my accelerator pedal purchase was a waste of time and money? I am not impressed with the Land Rover resources that can be found online, and as an owner I'd expect at least some access to some decent documentation. Thanks all for being here...

An update...
Since December I've had the top engine cover (on intake manifold) off to facilitate testing, have checked my accelerator pedal assembly against the replacement and they are both the same, no problems with them. The problem has not come back, so will pull apart some of the electrical connectors in the spring to ensure no corrosion, this is a strange one!

I posted this answer in another thread but can't remember where, I cleaned the main battery ground (under the front passenger side front fender liner) and also installed another battery ground cable from there to the engine block, have not had an issue since! Looking through all the electrical (schematic) drawings I note that all these APP and throttle body voltages are referenced to a common ground, so if there is something wrong with the reference grounds, then strange things will happen. That's the story for now, if anything else happens I'll re-post.

I can't believe it but as I was returning from Costco I started slowing for a red light with my foot on the brake pedal and was nearly stopped when it happened (sudden acceleration) again! I threw the transmission into neutral, engine revved up to limiter and then came back down to idle at about 800 RPM, I guess I'll do some more research, this is an aggravating and frustrating problem, they call me the "card fryer" at work so I'll do the "Fiat" thing ("Fix it again Tony") again...
