The only way the spare is going to be significantly larger than the other 4 road tires is if you run them right down to the cords and then try installing your spare.
Even if the tire size was hugely different the open diff will compensate for it.
The diff will just think that you are turning a really long corner.
Now if you had a all wheel drive car then no, all four tires must be the same size otherwise you will burn out the clutches in the diffs and transfer case.

And as for caring for the spare tire...this is why your uncle Spike says to check the tire PSI and remove the lugs that hold it on and coat the threads with anti-seize every 6 months.
