I would bring spare: Water pump, radiator, head gaskets, fuel pump, crank sensor, atleast 1 front and 1 rear hub (maybe 2 front hubs) atleast 2 spare front propeller shafts, (I would swap/rebuild one 1/3rd of the trip in, then swap/rebuild again at 2/3rds. hey better safe than sorry) drivers window regulator, drivers door latch, a spare steering box..... Actually you could just tow a parts car the whole way if you really wanted to...

Chances seem to be pretty high it'll keep running until you get where you're going if it's going to be getting there in the first place, and if not you'll find a shop for parts along the way.
I'd pack an extra ipod full of loud driving music, and maybe a hot girlfriend who won't complain about all the windows being down.

I put a lot of miles on my truck and always have my kit in the back, which thankfully I have yet to use. Have all the normal safety stuff, jumper cables, first aid kit, flares, etc.. Plus, a spare serpentine belt, a tow strap, gorilla tape, super glue, screw drivers and wrenches, bag of wire ties. That's about it.

Opps, I lied. I have used the wire ties and tape ;-)

Is this an offroad or onroad journey? Just have to ask as you have an 04 with 60K, which in all honesty is just as solid as anything on the road.
If it is onroad, a AAA card in case something happens plus fluids.
If offroad, well, all the other stuff to DIY on the trail.
Either way, beef jerky, coffee and altoids

there will be some offroad time, but nothing to rough since i'll be carrying 500 lbs of crap with me. (i'm moving)

Now this sounds interesting. Can you post a write up of what you're doing for the conversion?

Maybe I missed it somewhere in the thread, if so I apologize, but do you have steel bumpers and sliders? If not, then I'd recommend packing that hi-lift jack at the bottom of your load and leaving it there. Your bottle jack is far more useful and safer if you don't have the proper points to hit on the truck. (I have a hi-lift zip tied to bottoms of my front seats and it's never left the truck because the only place to jack on is the tow hitch. Sliders w/ hi-lift jack points are on order...)

Something else I always have handy: toilet paper and immodium.
That has saved my *** many times
