Default modulator fix, three amigos are back

I worked on the svs and did option B. I had constant three amigos and my mechanic had to remove the three amigo with his test book after option B.
one week later, they are back and it looks like a hard fault. My switch tested fine so figured it was in that two pin socket. Any other idea why the amigos are back. I checked my wiring to make sure ground was grounded too.
Thanks

Read the code again. And recheck your ground. Some guys had had to create a new grou d for it to work.

Default Removing hard fault

Is there any way I can remove a hard fault with my code reader? It appears after testing I have continuity between the engine block and the ground screw.
