Default Symptoms of a bad O2 Sensor

If I have super low values from my upstream sensors, .006-.006, and downstream ones are still in the .400 range does that indicate a bad sensor?

Sorting out a high initial idle and rough running issue.

They are either dead or super lean, but since your ds sensors are middle of the road my guess is dead. Check them with a elm327 and the torque app graph running on a cold start. cold start they shoudl immediately go to 1 volt for the first 10-20 seconds, then start falling till they start cycling. If they don't go to 1 volt on cold start - dead or wire is cut.
