How old is the PCV ? I have read stories of those opening up letting too much oil in and fouling the plugs and clogging up causing a build up of crankcase pressure causing reverse ring blow by.
Seems like a long shot but it seems you have covered everything else.

I switched out the PCV with my V6, no changes. This is after running the vehicle for a fair while and a test drive. Then after doing a few favors for people I got my hands on a brand new fuel pump and threw that in the truck on the advice of a friend. Runs better, but still not great.

Although, yes... There is some significant water ingress on the passenger side. Such that after a heavy prolonged rain, when you open the door water pours out of the passenger side. I have tried to find the source of that, all the usual places and even sitting in the truck for an hour during the heaviest portion of a rainstorm, constantly inspecting the usual places and checking the door seals, fuse box, sunroof, etc... Note, I only realized this was a problem after the vehicle has been parked for a while.

I'll start tearing into that this week and see if I can make sense of anything. I had this issue with my other vehicle, but it only caused some intermittent key remote issues and random car alarms. Go figure.

I see you "cleaned" the EGR valve... have you ever replaced it? I had misfire issues with my 4.0L V6 and completely cleared up after replacing it with new. Same on my old Chevy pickup. They do need replacing at some point as part of maintenance. Just my .02 worth...

OK, have you verified the voltage to each of the injectors, and are all of them working? Like the coils packs you could try swapping them around is se what happens. Then, you can remove the injectors and test them to see if they have a good spray pattern?
If you have determined that the spark coils are good, the plugs are not oil fouled, the fuel pressure is good, the throttle body is clean, you may be down to the injector not consistently delivering the fuel.
Keep us informed of your progress, were not ready to give up yet.
