Not sure if this will correlate to your hardware version. This is from the ECU info via GAP tool.

So, the Fuel System monitor does not change immediately at startup even though it’s called ‘continuous.’ Documented for posterity.
Here are some screen shots. The next to last counter (Secondary O2 sensor monitor) turned from 3 to 4 and then all monitors were ready.



I think my take-away from this is I need to just be patient. I thought after a couple days of regular driving I would be read for I/M, but several things still show as "INC" in my OBD scanner. I have 30 days to complete my test, so I'll just keep on trucking for awhile before worrying too much. . .
