Time to get real, quit thinking good gas or better oil will fix it, it won't. At your mileage, it is safe to say you have a serious valve train issue. I would start with doing an oil pressure test to make sure that old pump is still up to the job, then it is really time for a semi-engine overhaul, oil pump, timing chain, cam, lifters, push rod and a new set of rocker shafts and arms, cause they are worn out.
By the way, it won't be sleeves if you are not adding coolant and have none in the crank case.

Do that before anything else and see what happens, worse case you're out the cost of a oil change.
No need to cut off your arm because you have a hang nail.

I was hoping that 146,000 miles was not a tired engine. I will see if this last oil change with rotella T 15-40 will continue to make a difference and hope that a top end job is not required for a while. Will do a oil pressure test asap( any quick way to do this?). No sign of coolant loss or in oil.
