3 Amigos + codes.
My 2004 SE7 has been running fine. Started it up yesterday and the HD + ABS + TC warning lights are all on. The ABS modulator was replaced at the dealer about 6000 miles ago for the same issue (Nov. 2011).
Truck is running well. Ultraguage reader is showing the following codes...
P0300
P0308
P0304
P0303
P0306
Any advice?

How many miles on your D2? It looks like you have multiple issues going on. All the codes are related to misfires on various cylinders. The three amigos needs to be read separately with a code reader that will give you the ABS codes. You are using an ultra gauge, it will not give you the ABS codes. I would be pretty angry with the stealership that replaced the ABS.

I brought the car to my dealer who just told me there are 2 shuttle valve failure codes and he is recommending that the ABS Modulator be replaced to the tune of $1500 (even though it was replaced 6000 miles and 18 months ago). Warranty is only good for 12 months.
Anyone have any thoughts on this? I am a little in the dark. Not sure what the typical life is from an ABS modulator.
I welcome any thoughts.

As for the ABS, it is time you get involved, get the actual fault codes, fix and clear them yourself for only $200 or less.

There are 87,000 miles on the truck.

Ok, The lights were cleared at the dealer and returned about a week later.
I got the RSW OBD plug tool and it is showing this error.
016:Shuttle valve long term fail
That is the only thing that it displays and the truck seems to be running well.
Any advice would be great.

I have the same 016:Shuttle valve long term fail fault code. In the past I replaced wheel sensors and got rid of those codes. Did the Recommended shuttle fixes with Option B.
I can erase the code and the truck runs for 2-3 days without any Code then comes back with 016 code.
Any help would be appreciated.

One thing I found when I did the option B shuttle valve work was that I had to relocate the new ground a couple of times. I ran mine all the way over to a ground on the opposite side, to a ground bolt near the fuse box. I put the wire into the group that runs on the back edge of the engine compartment to keep it out of the way. Hope this helps.
