The '99's did come with the nipple.
It could very well be that the transfer case was changed at some time prior to your purchase of the truck.
The original driveshaft had a zerk fitting on the front u-joint from the factory. It's the two in the back that are typically the problem because they don't have zerks and are not serviceable.

Sucks that the truck doesnt have a cdl, but I plan to sell it anyway.

Did you get the housing with the bent arm...makes the install easier.
yes it should move side to side. You will see the end of the shaft moving in and out of the shifter housing. took me about 1/2 an hour of working the shifter around soaking it with pen oil to get it moving.

the housing on top of the transfer case. where the hi/lo range arm connects to held in by 6 bolts. if you go back a few posts there is a link to pics of the set up. the stock D2 cable connects to a straight arm and if you use this you will have to bend the link rod to work.

finally got back to freeing up the rod
leave to our friends across the pond using dissimilar
metals...steel shaft in a alum. housing, that's why they bind
honed the hole out by locking the shaft in my cordless and spinning
while appling PB Blaster the coated everything with green grease
now it's better than new
quicksilver 101 is the best grease for this application, used it on boats for years and have never had a problem with seize

X2 on verifying CDL presents and not assuming anything based on the model year and build date. I had an early '01 that came off the line in 2000. I sourced a D1 shifter without confirming, opened the center console, and no nipple. Had to add the front output assembly. After some research on the VIN by Will Tillery, it had come with CDL, but was swapped out during warranty period by LR for the PO with a non-CDL transfer case.
