Looks like it died from lack of grease, look at all the red rust where the needle bearings run....dry and rusty.

Yes...

Its also the one with the zurk port... That's the weakest spot on that ujoint. But no greasy, like it was bypassing that cap.

When you grease a u-joint you have to make sure fresh grease comes out of all four caps. Sometimes you waste a ton of grease before that happens, but otherwise the job is not done.
