Gear Shift Problem on Landrover D2

I have a 2000 model Landrover Disco 2, Turbo Diesel, Auto.
I am presently experiencing a problem whereby, especially from a standing start, with the gearshift in “Drive”, it seems to remain in second gear with the revs going upto as high as 3800. I then seem to have two choices – release the accelerator completely or kick down to make it change into 3rd and Drive. This problem does not manifest itself all the time but is intermittent. The Airflow meter was “suspect” and was replaced a week ago. No fault codes show up on the diagnostic computer. Transmission has been checked for “slip”, even uphill, and appears to be fine. Filter and T/fluid replaced periodically and as late as last week, but fault still persists occasionally.
