Update: I had the engine block tested and it failed twice. There is a cylinder crack and the engine is toast. So that is why the overheating kept occurring even after bleeding. I must have driven the truck too long while the sloshing sound in coolant system was happening. I can't think of any other reason.

Actually my guess would be the sloshing was an initial symptom of the damaged engine, exhaust was being "injected" inline with the coolant and must have been getting pretty bad.
