When was the last time you rotated your tires or had the brakes done, someone did not tighten the lug nuts down to 102 foot pounds of torque.
You should remover both the rear tires and inspect the rear lugs to make sure they were not damaged by having this happen. If any thread were damaged, get the lugs replaced.

When we sat down for dinner tonight and said prayer I had to give a special thanks cause I don't think we would be here right now if someone wasn't holding that tire on for us.
Not sure if you did the brakes, or a shop, but sounds like that wheel was put on wrench tight, then they forgot to come around and torque them afterwards on that wheel. Be sure to now check ALL the wheels. Glad you are all safe...
