Blown HG, cause?

Well the arp stud nuts came off between 45-60 ft lbs. Seems low to me compared to stretch bolts but I need to look up the spec. The studs themselves were loose and could spin out by hand. Also the top hats are about 0.003” low. I’m dropping the heads off tomorrow morning at the machinist. Here is a photo of the blow out:

The ARP docs say 70Lbs at the end.
