P0193 Code
That's certainly what it looks like with the diaphragm built into the top of the cover. But I thought there was an environmental regulation that required those fumes to be recirculated through the combustion chamber?
I haven't taken one apart but I'm sure it recircs normally for the reason you mentioned, the diaphragm might just be a pressure relief if there's more blow-by than the intake can handle (I think I've heard it relieve when cold at high elevations). First noticed that on a cold day at 8000ft, but it's gotten more common over the years so maybe more blow-by as the motor ages?

There is a passage way in the cylinder head thru the timing chain cover and to the other side of the cylinder head and then into the intake air path of the engine

If those pics are your LR2, you have some clean valves! Which fuel cleaner do you use?

I buy mostly chevron gas since techron is pretty well accepted to be good stuff (philips 66 also used to have techron but I don't think it does any more). Any designated "top tier" gas brand will have more cleaning additives, so at the very least go with that if you're planning on keeping the car for a while.

- Top Tier Gasoline "it has a Techron like cleaner in the fuel no need to add more"
- PCV changed at about 100,000 miles, it wasn't broken (because I am a bit paranoid)
- Spark plugs Denso Iridium changed every 50,000 miles (did I mention that I am a bit paranoid)
