I'd be looking at all the piping from the egr to the intake manifold. That Y piece that broke off your throttle body looks almost completely obstructed.

"Mechanically" remove as much as one can get to. Carefully without dislodging any of it and sending it into the intake/engine. To get the manifolds entirely clean is to remove them, probably not worth the cost.

I just downgraded from my 2018 TD6 to a newer 2022 gas “RDynamic”. So I can stop following this and other similar tales of woe. Very disillusioned with all the diesel/DPF mess here in California - my dealer just replaced the whole DPF/DEF system yet again - I got just over 1000 miles on the last DPF - and I managed to get it traded in while the CEL was off … just before emissions warranty expired. Didn’t get much on the trade-in of course.

So I’ll still be reading about puddles in the footwells but hopefully never will see those dreaded DPF initials again (did I tell you I got me a 2000 F-350 7.3 diesel with 120gallons in the bed and not a single piece of electronics or software between engine and it’s 3” tailpipes …).

Same for me. After so many issues with the my new 2017 TD6, I “upgraded” by trading in for a new-to-me 2018 gas in 2020. Has been a relative dream. Had the potential troublesome plastic water pipes changed to metal to forstall that problem. Otherwise, has been trouble free at 120k miles.

Could be a def doser/injector clogged. Easy to access and clean. Common challenge with diesels in general .
